Job summary

REM Community Services, a part of Sevita, is seeking a Direct Support Professional to provide community-based support in residential settings and periodic programs. This role is non-exempt and hourly, with opportunities for full-time or part-time hours.

Responsibilities include assisting with daily living activities, health maintenance, medications, and community integration, while maintaining records and safety standards. We value compassion, reliability, and teamwork.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization and development.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping.
• Accurately document progress and activity; review records and logs to stay informed of changes in service plans.
• May accompany individuals to medical appointments and relay orders to providers.
• Administer or support self-administration of medications as assigned; document delivery of medications.
