Job summary

REM Community Services, a part of the Sevita family, seeks compassionate Direct Support Professionals to assist individuals in residential and community settings. You will deliver care per individualized plans, support daily living activities, and promote health, safety and independence.

Ideal candidates are 18+, with a high school diploma or equivalent and some experience in human services. You will document progress, maintain confidentiality, and collaborate with care teams to help individuals

Qualifications

• Direct care and support responsibilities as described in the job posting.
• Documentation of progress and maintaining confidentiality per policy.
• Ability to work with individuals with intellectual and developmental disabilities.

Responsibilities

• Deliver support per individualized service plans, including socialization and health maintenance.
• Assist with daily living activities: personal care, housekeeping, transportation, and community integration.
• Accurately document progress and maintain confidentiality.
• Maintain relationships with individuals, families, guardians and case managers; follow Sevita standards.
• Report abuse or neglect and medical/behavioral incidents per policy.
