Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) in Oxford, PA to provide hands-on, person-centered support for individuals with IDD in home and community settings. The role emphasizes dignity, independence, and community inclusion through individualized service plans and collaboration with an interdisciplinary team.

The DSP will assist with daily living activities, meal prep, mobility, and medication support as authorized, while documenting care and participating in trainings.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support for individuals with IDD preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered care in home/community settings.
• Assist with daily living activities and health/safety monitoring.
• Support meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and document observations.
• Maintain accurate documentation and participate in trainings and team meetings.
