Job summary

Sevita is a leading provider of home and community-based health care. We seek caring individuals to join our D&S Community Services team to assist participants in achieving independence and enjoying meaningful daily activities.

As a caregiver, you’ll build real relationships, support daily living needs, and collaborate with supervisors and peers to deliver person-centered care in the community.

Qualifications

• High School Diploma or equivalent.
• Six months of experience in human services, direct care, or care coordination preferred.
• Skills include communication, adaptability, multi-tasking, teamwork and time-management.

Responsibilities

• Assist people with their personal hygiene and support individuals with behavioral challenges and other complex needs.
• Help participants be part of the community and engage in activities they enjoy.
• Provide compassionate, patient care and build meaningful relationships with participants.
• Support participants with developmental goals like budgeting, exercise, and nutrition.
