Job summary

365 Health Services seeks a Direct Care Worker to provide in-home personal care to clients under the supervision of a licensed nurse. You will follow care plans, assist with daily activities, and ensure safety and dignity during transfers and mobility tasks.

The role requires strict adherence to infection control, confidentiality, and documentation standards. We offer weekly pay, diverse shifts, and comprehensive benefits in a home-care setting.

Qualifications

• Performs or assists patient with personal care as prescribed in the plan of care.
• Practices Universal Infection Control Precautions in all contacts with patients.
• Performs home maintenance tasks as prescribed in the care plan.
• Demonstrates safe transfer, ambulation and positioning techniques.
• Maintains confidentiality of all patients and families in line with HIPAA.

Responsibilities

• Assist patients with daily activities and personal care.
• Follow the care plan and report to RN or Director as needed.
• Maintain infection control standards during all patient interactions.
• Document care provided in a timely manner.
• Adhere to agency policies and HIPAA guidelines.
