365 Health Services is seeking a Direct Care Worker to provide in-home personal care and support across multiple shifts. You will assist with hygiene, mobility, nutrition, and daily activities as outlined in the patient care plan, while upholding HIPAA and infection control standards.

Qualifications

• Must be able to perform personal care tasks as prescribed in the plan of care.
• Complies with universal infection control precautions and HIPAA guidelines.
• Capable of documenting care provided and patient responses in a timely manner.

Responsibilities

• Assist patients with personal care activities per the care plan.
• Provide home maintenance assistance as prescribed, including light housekeeping and meal prep.
• Practice safe transfers, ambulation, and positioning techniques.
• Document care provided and patient/family response in a timely manner.
• Maintain regular attendance and adhere to agency policies.
