365 Health Services is seeking a Direct Care Worker to provide in-home care to clients in the Montgomery County area. The role involves assisting with activities of daily living, mobility support, and maintaining a clean, safe home environment. You will follow the care plan, report changes to the RN, and document care provided.

Qualifications

• Provides personal care and assistance as prescribed in care plan.
• Maintains infection control practices in all patient contact.
• Performs safe transfers and ambulation techniques.
• Documents care provided and patient responses promptly.

Responsibilities

• Assist patients with personal care tasks (hygiene, mobility, elimination).
• Assist with home maintenance activities per care plan (light housekeeping, meal prep).
• Document care provided and patient status in charting.
• Maintain confidentiality and HIPAA compliance.
• Adhere to agency policies and safety standards.
