Job summary

Team Select Home Care is seeking a Direct Care Worker to provide non-medical personal care in clients' homes and assist with daily living activities. You will follow the Assignment/Service Plan, maintain confidentiality under HIPAA, document visits, and report changes to the DOO/designee.

The role emphasizes compassion, reliability, and adherence to state regulations. Training and ongoing learning are encouraged, with competitive pay and flexible scheduling.

Qualifications

• Sympathetic attitude toward care of the sick
• Demonstrated ability to read, write, and carry out directions
• Good verbal and written communications skills required

Responsibilities

• Helps the consumer with non-medical personal care including bathing, nail and skin care (no nail cutting) and daily hygiene
• Assists with transfers and transfer devices including lift devices per state regulations
• Prepares nutritious meals/snacks and assists with feeding
• Assists with non-medical health maintenance activities and routine wellness services
• Escorts/transports consumers to medical appointments when authorized by office staff
• Provides companion services to assist functionally impaired individuals
• Maintains confidentiality of client information and HIPAA compliance
• Completes documentation for each shift/visit with consumer signature
