Job summary

365 Health Services is seeking a Direct Care Worker to provide in-home care to clients in Eastern PA. You will assist with personal care, mobility, hygiene, and medication reminders, following the plan of care and HIPAA guidelines.

We offer weekly pay, benefits, 401k, flexible schedules, and a supportive team. Work location is in person with various shift options including day, evening, night, and overnight.

Qualifications

• Must be able to assist with personal care activities as prescribed.
• Ability to follow care plans and report to RN as needed.
• Maintain patient confidentiality and HIPAA guidelines.

Responsibilities

• Performs or assists patient with personal care activities as prescribed in the plan of care.
• Practices Universal Infection Control Precautions in all contacts with patients.
• Performs or assists patient/family with home maintenance activities as prescribed in the care plan.
• Practices safe transfer, ambulation, normal range of motion and positioning techniques.
• Respects patient confidentiality and autonomy at all times.
• Documents care provided and responses from patients/families in a timely manner.
