Job summary

PACS is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care and services to residents according to care plans. You will identify residents, assist with daily activities, document care, report changes to supervisors, and maintain a safe, caring environment across shifts.

CPR certification is preferred. The role requires adherence to confidentiality, proper reporting, and teamwork to ensure high-quality resident care within the facility.

Qualifications

• Education and/or experience must possess, as a minimum, an 10th grade education.
• Language skills: ability to read, analyze and interpret general business materials and write reports.

Responsibilities

• Identify residents before administering treatments or meals.
• Record entries on flow sheets, notes, and charts.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of protected health information and report incidents.
• Assist residents with daily functions and personal care needs.
• Ensure rooms are ready for residents and assist with transfers or discharges.
