Job summary

Qualifications

Responsibilities

• Assist residents with activities of daily living per care plans and policies.
• Assist with mobility, positioning, and transportation as needed.
• Coordinate dining room services, meal delivery, and intake documentation.
• Record resident weights and report changes to nursing staff.
• Document progress on flow sheets and communicate barriers to care.
