Job summary

Responsibilities include ADL assistance, meal support, documentation of care, and participation in care plans. Must have CPR/BLS and at least one year of experience in long-term care. Position available as full-time or part-time on various shifts with compassionate patient care as a priority.

Qualifications

• Must be CNA with good standing in state and CPR/BLS certified.
• High school diploma or equivalent required.
• Active CNA certification required (state).

Responsibilities

• Provides supportive services to nurse(s) and other staff as needed.
• Assists resident with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving, positioning, and transporting residents.
• Coordinates dining room services at meals: setup, tray delivery, feeding assistance, and documenting intake.
• Delivers nutritional supplements and ensures intake is documented.
• Assists with weighing residents and reports weight changes to nurse/supervisor.
• Monitors skin condition and reports any issues to nurse/supervisor.
• Supports toileting program activities and completes daily flow sheets.
• Keeps staff informed of factors affecting performance and resident status.
• Reports changes in residents condition to nurse/supervisor.
