Job summary

Alta-Heights-Post-Acute in Nashville, TN is seeking a Certified Nursing Assistant to provide routine daily nursing care to residents per assessment and care plan. You will identify residents before procedures and assist with daily activities in a warm, respectful environment.

You will document care accurately, report changes to the Nurse Supervisor, maintain confidentiality, and cooperate with staff to ensure safety and comfort.

Qualifications

• Must have Certified Nursing Assistant license per state law.
• Minimum 10th grade education required.
• CPR certification preferred.
• Ability to read, write, and interpret basic instructions.

Responsibilities

• Provide routine daily nursing care per care plan.
• Identify residents using wristband/photo before procedures.
• Record entries accurately on flow sheets and charts.
• Report changes in resident condition to Nurse Supervisor.
• Assist with mobility, transfers, and daily activities.
• Maintain confidentiality and follow HIPAA guidelines.
