Job summary

Homestead Post Acute is seeking a compassionate nurse aide to provide routine daily nursing care per the resident care plan. You will verify resident identity, document observations, and promptly report changes to the Nurse Supervisor. Maintain confidentiality and follow safety protocols across shifts.

You will assist with daily activities, hygiene, meals, and transfers, ensuring a warm, respectful environment and accurate recordkeeping for each resident.

Qualifications

• Provide routine daily nursing care per care plans.
• Identify residents using wristbands or photo IDs when needed.
• Report changes in condition to the Nurse Supervisor promptly.
• Maintain privacy and confidentiality of resident information.
• Follow policies for blood/body fluid exposure and safety.

Responsibilities

• Greet residents, escort to rooms, and introduce them to roommates.
• Keep rooms ready, beds made, and admission kits available.
• Record vitals and nutrition notes as instructed.
• Assist with daily activities, meals, hygiene, and transfers.
• Observe skin integrity and report decubitus risk promptly.
• Participate in shift nursing reports and maintain warm environment.
