Job summary

Heritage-Woods-of-Dekalb-1 is seeking a dedicated CNA to provide individualized care per each resident’s plan. You will assist with Activities of Daily Living while promoting independence and dignity, and you will report health changes to the clinical team.

Responsibilities

• Deliver exceptional, individualized care according to each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Engage with residents and their families in a compassionate and meaningful way.
• Participate in educational seminars, staff meetings, and other learning experiences.
• Maintain confidentiality and privacy of resident information.
