Job summary

Qualifications

Responsibilities

• Assist in clinic operations and promote resident comfort and confidence.
• Provide personal care per care plan (bathing, grooming, assistance with feeding, ambulation, dressing, toileting, etc.).
• Document care in electronic health records for each resident.
• Assist with meal planning and preparation (Home Care only).
• Maintain confidentiality and HIPAA compliance.
• Triage requests by consulting the Nurse Supervisor or Physician.
• Follow safety standards and legal regulations.
• Support Salemtowne’s mission and values.
• Adhere to policies and report concerns.
• Perform additional duties as assigned.
