Job summary

Team Select Home Care of Colorado, LLC is seeking a dedicated CNA to provide skilled personal care and assistance with activities of daily living in patients' homes under the supervision of a registered nurse. You will report to the RN and work as part of a care team.

Responsibilities include executing the plan of care, helping with meals, mobility, medications self-admin, maintaining infection control, and completing visit documentation.

Responsibilities

• Document visit records timely per policy.
• Report changes to the RN promptly.
• Maintain infection control practices.
• Attend staff meetings and care conferences.
