Job summary

21st Century Home Health Services Inc. is seeking a dedicated Certified Home Health Aide (CHHA) for Grass Valley territory. You will provide patient care under RN supervision, including personal care, assisting with daily activities, and supporting the care plan.

A CA CHHA certification and BLS/CPR are required, plus a valid CA Driver's License. We offer a strong benefits package with 401(k) matching, generous PTO, health/dental/life insurance, FSA, and pet insurance.

Qualifications

• Valid CHHA CA Certification.
• Current BLS/CPR Certification.
• Valid CA Driver's Licence (License).
• Experience in home health care is highly preferred.
• Strong communication and rapport-building with patients and families.

Responsibilities

• Provide patient care under RN supervision according to the Plan of Care.
• Assist with bathing, grooming, oral hygiene, and mobility.
• Support patients with activities of daily living and personal care.
• Maintain clean, safe living environment and care equipment.
• Assist with medications as directed and ensure safety.
