Job summary

21st Century Home Health Services (21HHS) is hiring Full-time Certified Home Health Aide (CHHA) for El Dorado Hills and surrounding areas. You will provide in-home patient care under an RN Case Manager, following the Plan of Care and ensuring high-quality outcomes.

Required CHHA CA certification and BLS/CPR, valid CA driver’s license, and comfort working in a field-based role. 21HHS offers strong benefits including 401k matching and generous PTO.

Qualifications

• Valid CHHA CA Certification
• Current BLS/CPR Certification
• CA Driver's License is required or preferred (as per ad)

Responsibilities

• Provide personal care such as baths, grooming, oral hygiene, and changing linen.
• Assist patients with dressing, toileting, and feeding as needed.
• Provide skin care and prevention of breakdown; meet safety needs.
• Report changes to the RN Case Manager and document care.
• Work within an interdisciplinary team to coordinate patient care.
