Job summary

21st Century Home Health Services (21HHS) is seeking a Full-time Certified Home Health Aide (CHHA) to provide in-home patient care in Alameda County, Dublin, Livermore, Pleasanton and surrounding areas. You will work under an RN Case Manager to implement the Plan of Care and support patients in their daily activities.

Applicants should hold a CA CHHA certification, current BLS/CPR, and a valid CA driver’s license.

Qualifications

• Valid CA CHHA Certification
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience in home health care is highly preferred

Responsibilities

• Provide patient care under the direct supervision of an RN Case Manager according to the Plan of Care
• Provide personal care: baths, oral hygiene, dressing, mobility assistance
• Assist with activities of daily living and ensure patient safety at home
• Maintain clean, orderly living environment and document patient status
• Assist in feeding and medication self-administration under supervision
• Report significant changes to the supervising nurse and work with an interdisciplinary team
