21st Century Home Health Services Inc. is hiring a Full-time Certified Home Health Aide (CHHA) for Alameda County, Dublin, Livermore, Pleasanton and surrounding areas. You will work under an RN Case Manager to follow the Plan of Care and support patients in their homes.

The role requires CA CHHA certification, current BLS/CPR, and a valid CA driver's license, with a focus on compassionate, high-quality home health care and teamwork with an interdisciplinary staff.

Qualifications

• Valid CHHA CA Certification is required.
• Current BLS/CPR Certification.
• Valid CA Driver's License.

Responsibilities

• Provide patient care under RN supervision according to the Plan of Care.
• Assist with daily living activities including bathing, dressing, hygiene and nutrition.
• Maintain patient safety and proper use of medical equipment.
• Document care and report significant changes to supervising RN.
