Job summary

Catholic Charities, Diocese of Pittsburgh is seeking a compassionate Caseworker to join the Washington County Outreach Team. You will provide case management, help clients access services, and coordinate needed supports to reach practical goals.

With strong communication and documentation skills, you will assess eligibility, arrange services, and refer clients to community resources, contributing to relief for those experiencing hardships while working within a collaborative team.

Qualifications

• Associate degree in a human services field or equivalent is required.
• Minimum of one year in social services (internship acceptable).
• Must be able to travel weekly to the Greene County office.
• Proven ability to interact constructively with community agencies and resources.
• Excellent problem-solving, written and oral communication, and documentation skills.
• Proficiency with Microsoft Office products.
• Clearances (Act 33/34 and FBI) required.

Responsibilities

• Provides case management and service planning for assigned clients to help meet objectives.
• Assesses requests for service to determine eligibility.
• Provides or arranges needed supportive services to fulfill service plans.
• Documents all client interactions and coordinating information timely and accurately to ensure program compliance.
• Develops new approaches to better serve client needs.
• Ensures clients achieve their service plan objectives.
• Provides information and referral services to link clients with resources.
