Job summary

Crow Wing County is seeking a compassionate Case Aide to join our Adult Mental Health team in Minnesota. This role provides direct support to adults by helping assess client needs, coordinate resources, and carry out service plans to improve well-being and independence.

Under the direction of a Social Worker, the Case Aide builds positive relationships, monitors progress, documents services, and connects individuals with community resources and services.

Qualifications

• High school diploma or equivalent required.
• Experience in case work, assessment, intervention and planning is preferred.
• Strong written and verbal communication skills required.

Responsibilities

• Conduct client assessments and respond to service requests with appropriate actions.
• Execute service plans to address short- and long-term client needs and evaluate outcomes.
• Manage active caseload with counseling, advocacy, coordination and assessment.
• Maintain case records including releases, data, plans, goals, outcomes and referrals.
• Ensure compliance with agency financial parameters and proper documentation.
