Job summary

The Guidance Center is seeking a Case Manager in Michigan to provide comprehensive case management for children, adults and families. You will assess needs, build individualized plans, arrange services and monitor progress, with a focus on person-centered care and community supports.

Qualifications

• Bachelor's degree in a human services field.
• Qualified Mental Health Professional credential within 120 days of hire.
• At least one year of experience with adults with mental illness.
• Valid Michigan Chauffeur License with a good driving record.
• Ability to engage adults, families and supporters.
• Orientation toward community outreach and cultural sensitivity.
• Ability to work independently with strong organizational, verbal and written skills.
• Ability to handle crisis situations.

Responsibilities

• Conduct case management assessments to identify areas of need and service preferences.
• Gather and integrate information from all available sources; document history and needs.
• Use motivational interviewing to assess motivation and plan services.
• Assist consumers in determining eligibility for entitlements and supports.
• Provide information about services and resources to consumers and families.
• Promote utilization of group and support services as appropriate.
• Develop comprehensive individualized Plans of Service with the treatment team.
• Provide face-to-face interventions to each assigned consumer at least monthly.
• Conduct home visits and community outreach to engage clients and ensure access to services.
• Link consumers to community services based on needs and preferences.
• Conduct skill-building interventions with individuals and groups as indicated.
• Maintain regular contact with treatment team and other providers.
• Reassess service needs at least every six months.
• Coordinate with health care providers to ensure continuity of care.
• Coordinate crisis intervention and discharge planning with community supports.
• Facilitate transition from inpatient to community settings with follow-up.
• Conduct crisis planning and provide after-hours crisis information.
• Establish collaborative relations with local providers and organizations.
• Maintain a file of community resources and participate in supervision and meetings.
• Document all face-to-face and phone contact; meet productivity standards.
