Cornerstone Management is seeking a Care Provider/Resident Assistant to support residents with daily living and health tasks in our Minnesota community. The role emphasizes compassionate care, safety, and preserving residents' independence and dignity.

You must be 18+, follow individualized care plans, document progress, and communicate with the care team. Training is provided and HIPAA compliance is required.

Qualifications

• Must be 18+ to apply.
• Experience in senior resident services, health care, or customer service preferred.
• Ability to work flexible hours, including evenings or weekends.

Responsibilities

• Follow each resident's ISP and daily task sheets.
• Obtain vital signs and report deviations to supervising nurse.
• Provide personal care and assist with activities of daily living.
• Maintain a safe, clean, and respectful environment for residents.
• Document resident progress promptly and accurately.
• Communicate changes in resident status to supervisors.
