Job summary

Cornerstone Management is hiring a Care Provider/Resident Assistant to support seniors in our assisted living communities. The role requires compassionate care, adherence to individualized service plans, and collaboration with the team to maintain a safe, engaging environment.

Responsibilities include following ISP directives, documenting progress notes, assisting with activities, and maintaining confidentiality.

Qualifications

• Must be 18 years or older to apply.
• Experience in senior resident services, social work, health care, or customer service.
• Caregiver is trained to provide personal care and related services according to care plans.

Responsibilities

• Follows the resident's ISP as directed.
• Follows daily task sheets for each resident.
• Communicates with supervisor about changes in resident status or home situation.
• Maintains safety and cleanliness of the environment.
• Prepares and submits resident progress notes in a timely manner.
• Assists residents with activities and encourages engagement.
• Upholds HIPAA confidentiality and handles records properly.
• Attends bi-weekly in-service meetings and mandatory training.
• Reports equipment wear or malfunctions to the supervisor.
