Job summary

Cornerstone Management is seeking a dedicated Caregiver to join our Wisconsin team, providing personal care and support under individualized plans to ensure safety, dignity, and a positive daily experience for residents.

Responsibilities include following ISP, maintaining records, assisting with activities, and communicating changes in residents' condition. Prior experience with older adults is preferred; training is provided.

Qualifications

• Experience in senior resident services, social work, health care, or customer service.
• Strong communication skills with residents and families.
• Ability to follow instructions and care plans.

Responsibilities

• Follows the resident’s ISP as directed.
• Follows the daily task sheets developed for each resident.
• Communicates with the appropriate supervisor about changes in the resident’s status.
• Utilizes standard precautions and infection control procedures.
• Prepares resident progress notes and submits documentation.
• Assists with activities and encourages resident engagement and participation.
• Upholds confidentiality and HIPAA compliance.
