Job summary

Assurance Care & Support Services Inc in New Jersey seeks a dedicated Home Health Aide to support clients with daily living activities and personal care in their homes. You will follow individualized care plans, promote safety, and deliver compassionate companionship.

Responsibilities include assisting with ADLs, mobility and transfers, medication reminders, light housekeeping, and documenting care while upholding confidentiality and coordinating with the healthcare team.

Qualifications

• HHA certification or PCA training, or willingness to obtain.
• Previous experience in home healthcare preferred.
• Compassion, empathy and genuine interest in helping others.
• Strong communication and interpersonal skills.
• Able to follow care plans and adapt to client needs.
• Reliable transportation and valid driver's license if required for visits.
• Able to pass background check and drug screening.

Responsibilities

• Assist with activities of daily living (ADLs) per individualized care plan.
• Assist with mobility, transfers, and prescribed exercises.
• Administer medication reminders and ensure adherence to plans.
• Perform light housekeeping, meal prep, laundry, and safe environment maintenance.
• Monitor and report changes in client condition to supervising nurse.
• Provide emotional support and companionship to clients.
• Accompany clients to appointments, shopping, and activities as needed.
• Document care activities accurately and maintain confidentiality.
• Collaborate with healthcare team for holistic care.
