Job summary

Assurance Care & Support Services Inc. in New Jersey is seeking an empathetic Home Health Aide to assist clients with daily living activities, mobility, medication reminders, and housekeeping, following individualized care plans.

You will support emotional well-being, accompany clients to appointments, document activities, and collaborate with the health team to ensure safe, holistic care. A valid HHA/PCA credential and reliable transportation are preferred.

Qualifications

• Certification as a Home Health Aide (HHA) or PCA training program.
• Previous experience in home healthcare or a related field is preferred.
• Strong communication and interpersonal skills.
• Ability to follow care plans and adapt to the unique needs of each client.

Responsibilities

• One to One Care, Provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, based on the individualized care plan.
• Assist with mobility, transfers, and exercises as prescribed by healthcare professionals.
• Administer medication reminders and ensure adherence to prescribed treatment plans.
• Perform light housekeeping tasks such as meal preparation, laundry, and maintaining a clean and safe environment.
• Monitor and report changes in the client's physical and mental condition to the supervising nurse or healthcare professional.
• Foster a positive and supportive atmosphere, promoting emotional well-being and companionship.
• Accompany clients to medical appointments, shopping, and other activities as needed.
• Document care activities accurately and maintain confidentiality of client information.
• Collaborate with the healthcare team to ensure a holistic and coordinated approach to client care.
