Job summary

Assurance Care & Support Services Inc is seeking a dedicated Home Health Aide to support clients in their homes in New Jersey. You will assist with daily living activities, mobility, and medication reminders, while maintaining a safe, clean, and respectful environment.

You will follow individualized care plans, monitor changes, report to the supervising nurse, and accompany clients to appointments. Great communication, reliability, and compassion are essential for building trust and promoting

Qualifications

Certification as a Home Health Aide (HHA) or completion of a Personal Care Assistant (PCA) training program or willingness to obtain.
Previous experience in home healthcare or a related field is preferred.
Compassion, empathy, and a genuine interest in improving the lives of others.
Strong communication and interpersonal skills.
Ability to follow care plans and adapt to the unique needs of each client.
Reliable transportation and a valid driver's license, if required for home visits.
Ability to pass a background check and drug screening.

Responsibilities

Assist with ADLs (bathing, dressing, grooming, toileting) per care plan.
Assist with mobility, transfers, and prescribed exercises.
Administer medication reminders and ensure adherence to treatment plans.
Perform light housekeeping (meals, laundry, cleaning).
Monitor and report changes to supervising nurse or healthcare professional.
Foster a positive and supportive atmosphere.
Accompany clients to medical appointments, shopping, and activities as needed.
Document care activities accurately and maintain confidentiality of client information.
Collaborate with healthcare team for holistic care.
