This role emphasizes compassionate, client-centered care, collaboration with nurses, and accurate documentation. A valid HHA or PCA training is preferred, with reliable transportation and a background check required.

Qualifications

• HHA certification or PCA training completion, or willingness to obtain.
• Experience in home healthcare preferred.
• Compassion and empathy to improve client well-being.
• Strong communication and interpersonal abilities.
• Ability to follow individualized care plans.
• Reliable transportation and valid driver's license if required for home visits.
• Ability to pass a background check and drug screening.

Responsabilités

• One to One Care, provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, based on the individualized care plan.
• Assist with mobility, transfers, and exercises as prescribed by healthcare professionals.
• Administer medication reminders and ensure adherence to prescribed treatment plans.
• Perform light housekeeping tasks such as meal preparation, laundry, and maintaining a clean and safe environment.
• Monitor and report changes in the client's physical and mental condition to the supervising nurse or healthcare professional.
• Foster a positive and supportive atmosphere, promoting emotional well-being and companionship.
• Accompany clients to medical appointments, shopping, and other activities as needed.
• Document care activities accurately and maintain confidentiality of client information.
• Collaborate with the healthcare team to ensure a holistic and coordinated approach to client care.
