Job summary

Heritage of Woodlands is seeking a dedicated Caregiver to provide compassionate, professional care for residents in memory care settings. You will support daily living activities, follow care plans, assist with transfers and hygiene, and help maintain a clean, safe environment.

You will engage residents in personalized activities and document care and incidents as required. Strong English communication, reliability, and teamwork are essential; memory care experience is preferred, and ongoing

Qualifications

• High school diploma or equivalent.
• Memory Care experience preferred.
• Must complete 12 hours of continuing education – provided in-services.
• Must be fluent in English and read/interpret resident care plans and policies.
• Ability to complete incident reports and basic documentation.
• Must work with other staff towards resident care.
• Ability to report to work 15 minutes early for walk-throughs.

Responsibilities

• Implements and supports resident care plan and assists with ADLs.
• Provides physical care including transfers, baths, hygiene, toileting, meals.
• Changes bed linens, does resident laundry and keeps rooms neat.
• Takes vital signs at intervals and records measurements; reports changes.
• Facilitates resident activities and participates in care conferences.
• Engages residents in meaningful activities related to their interests.
• Completes incident reports and responds to emergencies.
• Performs other duties as assigned.
