Job summary

Sky Heart Home Care Services in Concord, NC is seeking a compassionate Care Transport Associate (Driver) to transport clients to medical appointments, pharmacies, and community locations with safety and respect. Responsibilities include assisting clients with boarding, maintaining a clean vehicle, communicating with care coordinators, and adhering to traffic laws and company policies.

Must pass background check and drug screening; CPR/First Aid and Defensive Driving training are available

Qualifications

Valid driver's license with no major violations in the past 3 years.
Must have own reliable transportation.
Ability to assist clients with mobility, including lifting and transferring as needed.
Strong punctuality, reliability, and time management.
Excellent communication and interpersonal skills.
Background check and drug screening required.
CPR/First Aid, PASS training and Defensive Driving training available through the company.

Responsibilities

Safely transport clients to medical appointments, errands, and other destinations.
Assist clients with boarding and exiting the vehicle, including mobility challenges.
Maintain a clean, safe, and well-functioning vehicle at all times.
Communicate effectively with care coordinators regarding scheduling and client needs.
Monitor and report changes in client condition or behavior to staff.
Adhere to traffic laws, safety regulations, and company policies.
Provide courteous, patient-centered service to clients and families.
