Job summary

Milestone is looking for compassionate Care Partners to support residents in our Assisted Living and Memory Care communities in Mesa, AZ. You will provide personal care, assist with meals, and relay changes to the Health & Wellness team to ensure safe, high-quality care.

Join a welcoming team dedicated to residents, families, and coworkers, with opportunities to grow. High School diploma and caregiving experience preferred; English communication and documentation are essential.

Qualifications

• High School Diploma or equivalent required.
• At least one year of caregiving or similar experience.
• Familiarity with Microsoft Word and Excel preferred.
• Ability to communicate clearly in English and complete documentation accurately.
• Strong organizational skills, reliability, and attention to detail.

Responsibilities

• Follow resident service plans in a friendly, respectful manner.
• Document changes in resident condition per policy and state requirements.
• Support Quality Improvement by identifying issues and informing leadership.
• Adhere to HIPAA, OSHA, and infection control guidelines.
• Report unsafe actions immediately and communicate with care team.
• Assist with meal service including dining room cleanliness.
• Foster teamwork and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned.
