Job summary

Milestone is seeking compassionate Care Partners to assist residents with daily living needs in Assisted Living and Memory Care communities. You will provide personal care, assist meals and dining, and inform the Health & Wellness team of changes in residents’ conditions.

The role emphasizes safety, individualized care, and collaboration with caregivers, families, and leadership to uphold quality standards daily.

Qualifications

• High School Diploma or equivalent is required.
• At least one year of caregiving or similar experience is preferred.
• Proficiency with Microsoft Word and Excel is preferred.
• Clear English communication and accurate documentation are essential.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per policy and state requirements.
• Support quality improvement by identifying issues and communicating them to leadership.
• Adhere to HIPAA, OSHA, and infection control guidelines.
• Communicate effectively with team regarding resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Foster teamwork and positive interactions with residents, families, and coworkers.
