Job summary

Milestone is seeking compassionate Care Partners to support residents with daily living needs in our assisted living and memory care communities. You’ll provide personal care, assist with meals, and communicate changes in residents’ conditions to the Health & Wellness team.

This role focuses on safe, high-quality, individualized care every day. We value teamwork, reliability, and clear documentation, with occasional weekend or evening shifts as needed.

Qualifications

• Strong interpersonal, verbal, and written communication skills.
• Ability to document accurately and maintain clear notes.
• Willingness to follow care plans and company policies.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per company policy and state requirements.
• Support Continuous Quality Improvement by identifying issues and communicating them to leadership.
• Adhere to all company policies and regulations including HIPAA, OSHA, and infection control guidelines.
• Promote a safe working environment and report unsafe actions immediately.
• Report any suspected elder maltreatment or abuse according to policy.
• Communicate effectively with fellow Care Partners regarding resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Foster teamwork, respect, and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned
