Job summary

Milestone in Mesa, AZ is seeking compassionate Care Partners to support residents with daily living needs in our Assisted Living and Memory Care communities. You’ll provide personal care, support meals, and communicate important changes in residents’ conditions to the Health & Wellness team.

This role emphasizes safe, high-quality, individualized care and teamwork with residents, families, and coworkers. Weekend/evening shifts may be required as needed to ensure coverage and continuity of care.

Qualifications

• High school diploma or equivalent is required.
• Experience in caregiving or similar role is preferred.
• Strong communication skills and ability to document accurately.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per policy and state requirements.
• Support Quality Improvement by identifying issues and communicating them to leadership.
• Adhere to HIPAA, OSHA and infection control guidelines.
• Promote a safe working environment and report unsafe actions.
• Report any suspected elder maltreatment per policy.
• Communicate effectively with fellow Care Partners about resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Perform other duties as assigned.
