Ohio's Hospice Dayton is seeking a Care Services Representative to be the first point of contact for patients, families, physicians, and referral partners.

In this full-time, in-person role, you’ll deliver compassionate customer service, gather information, route calls to the appropriate clinician or department, and document details.

Qualifications

• High school diploma or GED required.
• Excellent verbal and written communication.
• Able to multitask across multiple systems.
• Maintain confidentiality and professionalism in all interactions.

Responsibilities

• Serve as the first point of contact for incoming calls to the Care Coordination Center.
• Gather accurate information and connect callers with the appropriate clinician, scheduler, triage nurse, department, or team member.
• Provide information about Ohio's Hospice services and programs.
• Accurately document call details in designated systems.
• Monitor call queues and support service-level goals.
• Identify and elevate urgent or time-sensitive concerns according to established procedures.
• Assist with other departmental responsibilities and projects as needed.
