Job summary

Ohio's Hospice is seeking a Referral Intake Specialist to serve as the first contact for patients, families, referral sources, and care teams in the Dayton area. This onsite, full-time role involves handling calls, accurately gathering and documenting information, and explaining hospice services with empathy and clarity.

The ideal candidate will have strong communication, data-entry skills, a valid driver’s license, and the ability to travel as needed.

Qualifications

• High school diploma or GED.
• Strong computer and data-entry skills.
• Excellent communication and customer-service skills.

Responsibilities

• Answer incoming calls and inquiries with professionalism, empathy, and exceptional customer service.
• Gather, verify, and accurately document patient and referral information.
• Explain hospice services clearly and answer questions from patients, families, healthcare providers, and community partners.
• Maintain accurate, complete, and timely documentation of all interactions.
• Work collaboratively with team members and provide support to other departments as needed.
• Participate in quality-improvement initiatives designed to enhance the patient and family experience.
• Assist with onboarding and training new team members when assigned.
