View Point Health is seeking a Behavioral Health Case Manager in Covington, GA to deliver intensive case management for adults with behavioral health needs. You will develop service plans, write progress notes, and coordinate with Medicaid and insurers to ensure compliant care.

The role emphasizes group facilitation, assessments, and linkage to community resources, with responsibilities including transportation for client activities and ongoing collaboration with an interdisciplinary team.

Formación

• Bachelor's degree in counseling or related field from an accredited college or university.
• Two years of job-related experience; or one year at Behavioral Health Counselor 1 or equivalent.
• Note: Some positions may require certification or licensure in social service delivery.

Responsabilidades

• Provides intensive case management for clients with severe behavioral health disorders.
• Serves primarily in an Adult Day Services Program.
• Conducts face-to-face assessments and writes individual service plans.
• Writes daily progress notes aligned with treatment outcomes.
• Secures signatures and authorizations for services; ensures regulatory compliance.
• Monitors progress and adjusts treatment plans as needed.
• Coordinates with Medicaid, insurers, and CARF standards.
• Establishes linkages with community services for client support.
• Provides transportation for community outings to teach relevant skills.
• Participates in interdisciplinary case staffing and team meetings.
