View Point Health in Georgia seeks a Behavioral Health Case Manager to provide intensive case management for adults with behavioral health challenges. The role operates across multiple sites and coordinates with a team of clinicians and community partners to write service plans and monitor progress.

The position requires a 4-year degree and two years of experience in social services, plus the ability to transport clients and pass background checks. Travel is up to 5%.

Competenze

• Bachelor's degree in a related field from an accredited college or university
• Two (2) years of experience in social service delivery
• Ability to lift 20 pounds and pass background/drug screenings

Mansioni

• Provides intensive case management for adults with behavioral health disorders
• Writes and maintains individual service plans meeting standards
• Coordinates with community providers and oversees service linkage and monitoring
