Qualifications

• Master’s degree in social work, psychology, or related field.
• Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification.
• Current CPR Certification.
• Experience in case management or behavioral health settings.
• Certification in Case Management preferred.
• Willingness to undergo and pass background checks and drug testing.

Responsibilities

• Ensure quality, cost-effective care and develop personalized care plans.
• Coordinate and monitor implementation of care plans with providers.
• Provide case management services, including referrals and coordination of services.
• Advocate for clients to ensure timely and appropriate services.
• Maintain accurate client records and document in EMR system.
• Coordinate referrals between specialty providers and agencies.
• Provide crisis intervention and support when needed.
• Collaborate with hospitals, social workers, and other professionals.
