Job summary

You will perform safety checks, assist with activities of daily living, observe and report patient conditions, and support transport or referrals as needed.

Qualifications

• High school diploma or equivalent required.
• Six months or more experience working with the specific population of the facility preferred.

Responsibilities

• Demonstrate a sense of urgency related to patient safety and provide excellent customer services.
• Ensure well-being of patients and provide a positive, supportive environment.
• Conduct safety checks and ensure supervision at 15 minute intervals as needed.
• Document accurate clinical information in the patient's medical record.
• Assist in providing a safe, secure and comfortable environment for patients, families and staff.
• Interact with patients, observe behaviors and communicate observations to nursing staff.
• May assist patients with activities of daily living (toileting, bathing, dressing, grooming, meals, hydration, changing linens).
• May obtain vital signs, height and weight and document.
• Facilitate patient educational-rehabilitative groups on topics such as social skills and independent living.
• Engage patients in activities designed to support treatment goals.
