Responsibilities

• Assists nursing staff in delivering care and maintaining a clean, safe environment.
• Records patient data (vital signs, intake/output, weight) and reports findings to RN/LPN/LVN.
• Assists with activities of daily living, meals, bathing, grooming, and mobility.
• Supports patient positioning, ambulation, and use of mobility aids.
• Performs glucose monitoring, urine/stool collection, and other delegated tasks as trained.
• Communicates patient information clearly to care team and adapts to change.
