Job summary

Legacy Concierge is seeking high-quality caregivers who provide 1:1 non-medical care in clients’ homes, emphasizing communication, relationships, and person-centered care.

Schedule includes Sun-Wed or Thu-Sat, 8a-8p or overnights, with required license and health documentation. You will document care notes and coordinate with the RN Care Ambassador and team, while maintaining confidentiality and client safety.

Qualifications

• 5+ years caregiving or personal caregiving experience.
• Registered Home Care Aide (CA) in good standing (CA only).
• Ability to work independently and follow instructions.

Responsibilities

• Deliver 1:1 personalized non-medical care in the client's home.
• Support activities of daily living (ADLs) such as personal care, transfer assistance, bathing, dressing, incontinence care, medication reminders, hand hygiene.
• Support instrumental activities of daily living (IADLs) such as meal preparation, light housekeeping, transportation, companion care.
• Complete daily care notes accurately and promptly using agency systems.
• Care for clients with conditions such as Alzheimer's, dementia, Parkinson's, post-operative recovery, memory loss, COPD, CHF, etc.
• Communicate with RN Care Ambassador or Client Care Coordination team when status changes or issues arise.
• Maintain a reliable schedule and communicate changes per policy.
• Adhere to HIPAA and privacy policies; protect client information.
