Job summary

UMMA is seeking a Wellness Coordinator to coordinate comprehensive wellness services, improving access to integrated mental and physical health care for underserved communities. You will enroll clients, support outreach, and promote our Integrated Care Program in a culturally sensitive setting.

The role involves working in a medical clinic environment with some travel within Los Angeles, handling data entry, reports, and coordination of care while upholding HIPAA standards and excellent customer

Qualifications

• Minimum 3 years’ experience in similar role.
• Experience with, knowledge of, multi-ethnic and underserved communities.
• Commitment to providing exceptional Customer Service.
• Proficiency in Microsoft Word, Excel, Outlook and Access.
• Proficiency in data entry and basic analytics.
• Experience with electronic medical record systems (EMR).
• Strong communication and relationship-building abilities.

Responsibilities

• Enrollment assistance for public insurance programs.
• Support implementation of outreach and engagement activities.
• Manage client enrollment and follow up as needed.
• Listen, identify, and recommend resources and services to help meet patient needs.
• Promote the Integrated Care Program.
• Provide individuals with information on UMMA’s services and help promote wellness and healthy behaviors.
• Identify and recruit individuals/families from underserved communities into the program.
• Demonstrates commitment to best customer service practices.
• Identify and develop initiatives to promote patient satisfaction and advocacy efforts.
• Able to multi-task on various applications and client needs simultaneously.
• Data entry of Outreach, Education, Activities for ICP/RSR Program.
• Submit monthly report of enrollment activities and other reports as assigned.
• Work with individuals in DMH programs and history of dropping out of services.
• Case Management
• Other duties as assigned by department leads.
