Job summary

UMMA is seeking a Wellness Coordinator to coordinate wellness services and improve access to integrated mental health, physical health, and substance abuse care. You will enroll patients in public programs, assist with outreach, and promote the Integrated Care Program within a medically underserved community.

The role involves collaborating with diverse populations, maintaining HIPAA standards, and reporting enrollment activity.

Qualifications

• Minimum 3 years’ experience in similar role.
• Community health worker certification, eligibility, or commitment to becoming a CHW as part of role.
• Experience with, knowledge of, multi-ethnic and underserved communities.
• Proficiency in Microsoft Word, Excel, Outlook and Access.
• Experience with electronic medical record systems.

SUMMARY

The Wellness Coordinator is responsible for coordinating the provision of wellness services to increase access to culturally sensitive integrated mental health, physical health, and substance abuse treatment options.

ESSENTIAL DUTIES

• Enrollment assistance for public insurance programs.
• Support implementation of outreach and engagement activities.
• Manage client enrollment and follow up as needed.
• Listen, identify, and recommend resources and services to help meet patient needs.
• Promote the Integrated Care Program.
• Provide individuals with information on UMMA’s services and help promote wellness and healthy behaviors.
• Identify, recruit, screen and enroll individuals/families from the Under Represented Ethnic Population community into the program, including individuals/families that have a history of dropping out of services.
• Demonstrates commitment to best customer service practices.
• Identify and develop initiatives to promote patient satisfaction and advocacy efforts.
• Able to multi-task on various applications and client needs simultaneously.
• Data Entry of Outreach, Education, Activities for the ICP/ RRR Program
• Submit monthly report of enrollment activities and other reports as assigned.
• Working with individuals who are in Department of Mental Health (DMH) programs and have a history of dropping out of mental and medical services
• Case Management
• Other duties as assigned by department leads.

ENVIRONMENT

The environment for this position is medical clinic that is clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will. In addition, the incumbent may travel via personal vehicle or public transportation throughout the community.

PHYSICAL ACTIVITY

In the course of performing this work, the incumbent:

• Will spend substantial time standing, sitting, speaking and listening
• Must regularly lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds
• May travel via personal vehicle or public transportation to other facilities throughout Los Angeles

QUALIFICATIONS/ POSITION REQUIREMENTS

EDUCATION:

Typically, these skills are the result of completion of a combination of education and experience equivalent to completion of two years of college or several years of experience in a community clinic setting.
