• 1–2 years of supervised healthcare/mental health experience; psychiatric conditions experience preferred.
• Strong communication, clinical documentation, case coordination, and community resource linkage skills.

Responsibilities

• Conduct psychosocial assessments and develop individualized rehabilitation plans.
• Provide crisis intervention and care coordination with therapists, doctors, and case managers.
• Educate and support families on the treatment process and community resources.
• Attend care conferences and advocate for client needs across service systems.
• Supervise and train Direct Care Workers and Community Outreach Specialists.
• Review documentation for accuracy, compliance, and clinical effectiveness.
• Monitor client progress and ensure timely service delivery.
• Participate in program development, policy review, and staff training.
• Represent CTM at outreach events and collaborate with community agencies.

Connections That Matter, Inc. (CTM) is a licensed and accredited behavioral health provider in Maryland offering wraparound mental health and psychiatric rehabilitation services to youth and adults. Our Psychiatric Rehabilitation Program (PRP) focuses on community integration, daily living skills, and strengthening support systems.

Position Summary

We are seeking an experienced Rehabilitation Specialist to provide direct psychosocial services, clinical assessments, treatment planning, and supervision within our Psychiatric Rehabilitation Program. The ideal candidate will also assist with identifying and recruiting clients who would benefit from rehabilitative mental health support.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized rehabilitation plans.
• Provide crisis intervention and care coordination with therapists, doctors, and case managers.
• Educate and support families on the treatment process and community resources.

Minimum Qualifications

• Education: Master’s degree in Social Work or a related behavioral health field.
• License: Must hold a valid Maryland mental health license (LMSW).
• Experience: 1-2 years of supervised experience in healthcare or mental health; experience with psychiatric conditions preferred.
• Skills: Strong communication, clinical documentation, case coordination, and community resource linkage skills.

Additional Requirements

• Must pass FBI and Maryland background checks, CPS, judiciary, and sex offender registry screenings.
• Completion of a physical exam and TB screening prior to employment.
• Ability to travel within Baltimore and surrounding communities as needed.
