Job summary

Lutheran Family Services in Grand Island, NE, is seeking a Community Support Specialist to provide rehabilitative and support services to adults with primary mental health diagnoses. Qualifications

• Bachelor’s degree preferred in Psychology, Sociology, or related Human Service field.
• In lieu of a bachelor’s degree, two years of relevant experience or training accepted.
• Case management and crisis management experience preferred.
• Ability to work with people from diverse backgrounds.

Responsibilities

• Provide rehabilitative and support services to adults with major mental health diagnoses.
• Develop integrated treatment plans with clients and update every few months.
• Coordinate transportation to appointments and community activities.
• Discharge planning and goal setting with clients.
• Perform required assessments and document services per guidelines.
• Participate in PQI activities and ensure HIPAA compliance.
