Job summary

Lutheran Family Services in Nebraska is seeking a Community Support Specialist to provide and coordinate rehabilitative services for adults with mental health diagnoses. Hybrid work environment includes in-office, field, and remote duties.

The role requires collaboration with an integrated treatment team, case management, discharge planning, and ongoing documentation to support client progress. A bachelor's degree in a human services field is preferred, with relevant experience acceptable in

Qualifications

• Bachelor’s degree or higher in Psychology, Sociology, or related Human Service field preferred.
• In lieu of a bachelor’s degree, two years of experience or training acceptable.
• Case management experience preferred; crisis management exposure beneficial.
• Valid driver’s license, proof of liability auto insurance, reliable vehicle required.
• Ability to work with diverse populations and backgrounds.

Responsibilities

• Develop with clients integrated treatment team plans, updating every 3 months or sooner if needed.
• Facilitate implementation of clients’ goals, including community living, daily living, skills, health and safety.
• Discuss discharge goals and develop a discharge plan.
• Conduct required assessments and document services per guidelines.
• Arrange transportation for clients to appointments and activities in the community.
• Develop crisis response and safety plans; assess and respond to crises.
• Coordinate treatment with services from other agencies and providers.
• Advocate for clients and partner with service providers.
• Maintain a caseload and meet program billing requirements monthly.
